Enrollment in CAJA (Caja Costarricense de Seguro Social) is mandatory for all Costa Rica residents. Here's exactly how the process works.
When to Enroll
You must enroll in CAJA within 30 days of receiving your residency approval. We handle this step as part of our concierge service.
What You Need
- Your approved residency documentation
- DIMEX card (or proof that it's in process)
- Passport
- Proof of address in Costa Rica
Monthly Cost
CAJA premiums are calculated as approximately 9–10% of your declared income:
| Monthly Income | Approximate CAJA Cost |
|---|---|
| $1,000 | $90–$100 |
| $2,000 | $180–$200 |
| $3,000 | $270–$300 |
For retirees on the minimum Pensionado income of $1,000/month, you'll pay roughly $90–$100/month for comprehensive healthcare.
What CAJA Covers
- Primary care: General doctor visits, preventive care
- Specialists: Cardiology, oncology, orthopedics, etc.
- Surgery: All medically necessary procedures
- Hospitalization: Including ICU if needed
- Prescription medications: Free or very low cost
- Lab work and imaging: Blood tests, X-rays, MRI, CT scans
- Mental health: Psychiatry and psychology
- Dental: Basic dental care
- Maternity: Full prenatal, delivery, and postnatal care
What CAJA Does NOT Cover
- Cosmetic surgery
- Some experimental treatments
- Elective procedures may have very long wait times
- Choice of specific doctor is limited
How It Works in Practice
1. You're assigned a local EBAIS (community health center) 2. Start with your general practitioner at the EBAIS 3. They refer you to specialists as needed 4. Emergency care is available at any CAJA hospital 24/7 5. Prescriptions are filled at CAJA pharmacies
Wait Times
This is the most common concern. For non-emergency procedures: - General practitioner: 1–2 weeks - Specialist referral: 1–3 months - Elective surgery: 3–12 months
For emergencies, care is immediate.
The Private Healthcare Option
Many residents use a dual approach: - CAJA for major procedures, hospitalization, and expensive treatments - Private clinics for routine visits, shorter wait times, and doctor choice
A private doctor visit costs $50–$80, which many expats consider worthwhile for convenience.
